Step 3. In the Subject section, type “REQUEST TO DELETE MY ACCOUNT”.

Cancel your Account through their Website
Follow these steps below:
Step 1. Log into your Twitch account on the main Twitch website.
Step 2. Click on the drop-down menu in the top-right corner and select Settings.
Step 3. Click on DISABLE ACCOUNT which is located on the bottom section of the settings page.
Step 4. Enter your Account Password.
Step 5. Enter your reason for leaving your account (optional).
Step 6. Now, Click on DISABLE ACCOUNT.
Note, a disabled account is not permanently deleted, it can be reactivated at the instances of the owner of the account.
So if you want to permanently delete your account, you must follow the first option (send a mail to the company, asking them to delete your account).
